Correction d4: expiration date (remove ni and replace with na).The device was received for evaluation with unknown fluids on the unit.A visual inspection was performed which observed light scratches on the top plate in slightly used condition, main module cover contaminated and magnets are reversed on pump cover.A functional electrical safety test was performed and the device passed.Functional system tests were performed and the device failed the large & small volume occlusion, syringe occlusion system testing with multiple occlusion errors.The reported condition of damaged rotor door (pump cover) was verified.The cause of the condition could not be determined, however, reversed magnets on pump cover will cause reverse polarity and produce the occlusion failures.Magnets may have became dislodged from normal everyday use and re-installed incorrectly by customer in reverse polarity.The door with magnets will be replaced to resolve the issue.A service history review revealed no indication that the parts replaced during servicing caused or contributed to the reported event.Should additional relevant information become available, a supplemental report will be submitted.
