The subject device has been returned to olympus for evaluation.During evaluation, it was observed, the tube of the cleaning liquid was noted to be clogged with a white crystalline particle.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported to olympus, during preparation for use, the customer was unable to supply ¿enzyme lotion and alcohol¿ with the endoscope reprocessor.It was also noted, the device had yellow error light blinging with no error code.The customer confirmed that the endoscope that was reprocessed by the subject device was not used on a patient.There was no harm or user injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The customers complaint was confirmed.The customer noted that the defect (detergent conduit was clogged) happened after a long period of inactivity of this device.After the defect happened, the device was not used again.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely that the user did not perform necessary treatment prior to storage of the subject equipment for a long period of time.As a result, cleaning solution turned solid in the cleaning solution tube.The event can be prevented by following the instructions for use: "7.17 preparing the reprocessor for long-term storage." olympus will continue to monitor field performance for this device.
