During a percutaneous catheter myocardial cryoablation procedure to treat atrial fibrillation in the left atrium a polarsheath was selected for use.The tip of the balloon catheter was inserted in the sheath, the air was sucked when confirming the backflow of blood using a syringe.The instruction on how to use the device properly was given, blood backflow confirmation was not recommended, and it was requested to insert the balloon catheter while performing sheath perfusion at a high flow rate.Subsequently, the device was used without issues.Per physician comment, the device was used incorrectly.
