The guide wire was returned to csi.An extra spring tip was observed on the core shaft.Visual examination did not observe any fracture or damage on the guide wire.The extra spring tip was due to an outside supplier component issue.The reported event of guide wire fracture was not confirmed.The material inspection report for this guidewire lot number has been reviewed.No issues or discrepancies were noted during this review that would have contributed to the reported event.The device met material, assembly, and quality control requirements.Csi id: (b)(4).
 
Event Description
Following treatment of a severely calcified lesion, angiographic imaging revealed the tip of the viperwire advance guide wire fractured in the ankle in the posterior tibial artery.A non-csi support catheter was used to retrieve the fractured tip.A shepherd guide wire was used to perform balloon angioplasty.The patient was stable.
