Event Description
Medtronic received information regarding a navigation system being used during a spinal procedure.It was reported that there were significant and sporadic navigation behavior during a case that caused the site to abort the use ofnavigation and imaging system.It was reported that intermittently navigation would place the tools multiple feet from the patient and the navigation window would not have any scans present but when zooming out, the representative was able to see the scan.This issue was noted to occur more frequently when swapping between the tap and screwdriver tools.The representative (rep) on-site was able to work around the issue by going back to the images and re-selecting patient scans and restarting navigation although this had limited success of 1/3 times.It was noted that the fix would be temporary and led to multiple usage of the work-around throughout the procedure.It was also noted tha t while navigating, multiple instruments were checked on l5 and s1 to be accurate but at the s2 location, the orientation accuracy was significantly off by multiple inches.The rep checked tracking details and no other instruments were showing when the issue occurred.There was a second reference frame during the case but it was covered and outside of the camera field.The rep believed that the issue was connected to the problem reported on a previous case.There was no impact on patient outcome and the issue caused a less than 1 hour delay.
