Date of event and udi number is unknown, no information has been provided to date.Investigation including root cause analysis is in progress.A supplemental mdr will be filed as necessary in accordance with 21 cfr 803.56 when additional reportable information becomes available.
 
Event Description
It was reported that the unit was not respond to the "alarm test" button.The customer suspected led's (light-emitting diodes) were disconnected, and the speaker was malfunctioning.The customer attempted to induce the "low water" alarm and the "tubing disconnected" alarm and the leds (light-emitting diodes) did not respond nor was there audible alarm sounds.There has been no report of patient involvement or no observable clinical symptoms or a change in symptoms identified in the patient.

Additional information received via email.The problem was discovered during regularly scheduled maintenance.No patient involvement was reported.
 
Manufacturer Narrative
G2 email is: (b)(4).One device was received.Per visual inspection, light emitting diode (led) lights on printed circuit board (pcb) were damaged.Per functional testing, when the alarms were triggered, the speaker made no sound.The complaint was confirmed.The root cause was a faulty speaker and broken leds on pcb.It was unknown what caused the condition.The product's history records were reviewed and there were no non-conformances nor service-related issues that would have resulted in the reported complaint.Replaced pcb, o-rings, and reservoir gasket.Calibrated the device.The device passed all functional and delivery tests.
