Event Description
|
According to the reporter, when connected and working during hemodialysis, the hemodialysis machine became clogged and the blood cou ld not flow out properly.It was also stated that the treating physician reported that the catheter was clogging.It was also mentioned that the doctor said that the catheter should be changed because the catheter was wrong.The catheter was not repaired.There was no leak.The patient was scheduled to undergo thoracic x-ray a day after the event to verify the malfunction, to see why it was clogging and for catheter replacement.The event did lead to or extend patient hospitalization as long as more time to dialyze.

According to the reporter, when connected and working during hemodialysis, the hemodialysis machine became clogged and the blood cou ld not flow out properly.It was also stated that the treating physician reported that the catheter was clogging.It was also mentioned that the doctor said that the catheter should be changed because it was wrong.It was also stated that the catheter was installed in the private sector; it was purchased, but now the hemolysis service is provided by a government institution (imss), and they were the ones who indicated that the catheter was wrong, and it was possible that they might ask to change it, but they did not give their opinion.The catheter was not repaired.There was no leak.The patient was scheduled to undergo a thoracic x-ray a day after the event to verify the malfunction, to see why it was clogging, and for catheter replacement.Since there were days that the catheter had worked well and others that had failed, the doctor had it under observation to determine whether it was necessary to change it or continue with the catheter, and stated that the delay was also due to the patient's delicate health.The event did lead to or extend patient hospitalization as long as it took more time to dialyze.
