Event Description
It was reported that during use with bd vacutainer® multiple sample luer adapter the adapter broke in half.This occurred on 2 occasions.There was no report of patient impact.The following information was provided by the initial reporter, translated from french to english: luer adapters are used to connect the trasis carpule (containing fdg) to the patient's infusion.At the time of injection, a luer adapter split in two.The activity is lost as the capsule is unusable resulting in a delay of the injection and the monopolisation of a lead tank (injection device) during the decay time.This problem has already been encountered twice with the same batch in this department.
 
Manufacturer Narrative
H.6.Investigation summary: bd had not received samples or photos for investigation.Therefore, 10 retention samples from bd inventory were evaluated by torque testing and no issues were observed relating to breaks apart as all samples met specifications.Based on a review of the device history record for the incident lot, all product specifications and requirements for lot release were met.There were no related quality issues during manufacturing of the product.This complaint is unable to be confirmed for the indicated failure mode breaks apart.Bd was not able to identify a root cause for the indicated failure mode.H3 other text : see h.10.
