Medtronic received information that during the implant of this 25mm aortic mechanical valve, it was explanted and replaced with a mechanical valve of the same exact make and model.The reason for replacement was reported as every time the valve opened and closed, blood had leaked through the valve leaflets.It was also reported that the blade of the valve detached from the valve during the flow test process leading to surgical problems, as the broken blade was removed from the patient with a surgical device.The mechanical valve was inspected before attempting to be implanted. no additional adverse patient effects were reported.
