Event Description
It was reported while using a cortrak device there was a "pneumothorax with tube placement." no additional information was provided.Additional information received (b)(6) 2023 stated the decision was to obtain an x-ray after tube placement as required by the user facility.
 
Manufacturer Narrative
The investigation concluded per customer comment, " the only reason that i know is because, as a result, the decision was made that we have to obtain an xray after tube placement at all of our facilities, regardless of level of user competency." this is in alignment with the warnings in the cortrak 2 enteral access system operator's manual, "comply with institution protocol to determine the final location of a feeding tube tip.Neither cortrak 2 trace nor feeding tube centimeter markings should be used as a sole method of confirmation.Clinical observation and institution protocols must be part of successful feeding tube placement.".Therefore, due to the limited information, the root cause was determined to be "unknown: no root cause was identified".All information reasonably known as of 06-jul-2023 has been included in this health authority report.Should additional information be obtained, a follow-up health authority report will be provided.The information provided by avanos medical, inc.Represents all of the known information at this time.Despite good faith efforts to obtain additional information, the complainant / reporter was unable or unwilling to provide any further patient, product, or procedural details to avanos medical, inc.Avanos medical, inc.Has no independent knowledge of the event reported but is relaying the information that was provided by the user facility where the incident occurred.This product incident is documented in the avanos medical, inc.Complaint database and identified as complaint: (b)(4).This information is submitted pursuant to 21cfr803, in compliance with the medical device reporting requirement and should not be considered to be an admission that an avanos medical, inc.Product is defective or caused serious injury.
