Event Description
|
Information was received from healthcare provider (hcp) via a manufacturer representative regarding a patient who had l5/s fusion f ailure.It was reported that patient had s 1screw removal s2ai screw replacement due to s1 screw loosening s2ai set screw dislocation.Patient had l5/s fusion failure due to screw loosening and dislocation.There was no patient symptom reported after revision surgery.There were no further complications reported regarding the event.
