Event Description
|
It was reported that during the cardiac resynchronization therapy defibrillator (crt-d) implant procedure, upon insertion of the right atrial (ra) lead into the header, no clicks were heard and the wrench was spinning loosely.The physician fully retracted the crt-d setscrew with the thought it may be cross-threaded.Several attempts were made to reinsert the ra lead and engage the setscrew effectively.The crt-d setscrew appeared to tighten without any audible clicks but the wrench did seem to spin loosely.A second wrench had to be opened as the first wrench became loose in the handle.Repeated attempts were made to engage the crt-d setscrew and the device was inspected without any obvious visual abnormalities.At certain points the crt-d setscrew was unable to be loosen to get the ra lead out.All leads were also withdrawn and re-insertion of each lead was re-attempted.Again the right ventricular (rv) lead and left ventricular (lv) lead were engaged without complication but, the ra lead continued to be a problem.A decision was made to replace the crt-d for a new header.The rv lead, lv lead and ra lead remain in use.No patient complications have been reported as a result of this event.
