During the initial implant procedure, the left ventricular (lv) lead was unable to be connected to the header of the device.Upon investigation, it appeared that the seal in the lv port of the device was missing.The device was replaced, and the implant procedure was successfully completed.The patient was in stable condition.

The reported field event of lead connection issue was confirmed.The lv septum was found to be damaged upon receipt.Septum material was observed inside the lv (is-4) set screw hex cavity, and the set screw was slightly stripped.The connection issue was consistent with having occurred during the procedure.The device was above elective replacement indicator (eri) when received.Telemetry, impedance, sensing, pacing and high voltage (hv) output functions of the device were tested and found to be normal.
