It was reported a patient underwent a revision procedure for unknown reasons.The cup and liner were removed and replaced.Attempts have been made and no further information is available.

(b)(4).This follow-up report is being submitted to relay additional information.An e-poly 36mm +3 hiwall lnr sz23, part#: ep-108323 from lot: 272260, was returned and evaluated against the complaint.No markings were observed on the returned liner.The liner was returned in 3 pieces.The pieces of deformed and covered in a dark debris.Dhr was reviewed and no discrepancies related to the reported event were found.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
