The healthcare professional reported the patient presented with middle cerebral artery (mca) stenosis underwent a stent placement procedure.During the procedure, the physician tried to release the stent, a 4.5mm x 22mm enterprise® vascular reconstruction device (enc452212 / 7338037) and the distal end of the stent became partially deployed, but the proximal end of the stent was impeded in the tip of the microcatheter (unspecified competitor brand) and could not advance or be withdrawn. The physician removed the stent and the microcatheter from the patient¿s anatomy and switched to new devices to complete the procedure. The patient was reported to be in stable condition. There was no report of any negative patient impact. On (b)(6) 2023, additional information was received. The information indicated that when the stent was removed from the patient, it was still on the delivery wire. There was no appearance of damage on the stent / stent delivery system.Nothing unusual was noted about the system prior to use.A continuous flush was maintained through the microcatheter; the microcatheter used was a headway® 17 microcatheter (microvention). It was replaced with another headway® 17 microcatheter. The replacement stent was another 4.5mm x 22mm enterprise® vascular reconstruction device (enc452212).The information confirmed there was no negative patient impact. The reported issue did not result in any delay.
