The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.The reported patient effects of thrombosis, angina and dissection are listed in the xience skypoint everolimus eluting coronary stent systems instructions for use as a known patient effects of coronary stenting procedures.A conclusive cause for the reported patient effects, and the relationship to the product, if any, cannot be determined; however, the subsequent treatment appears to be related to the operational context of the procedure.There is no indication of a product quality issue with respect to manufacture, design or labeling.
 
Event Description
It was reported that the procedure on (b)(6) was to treat a lesion located in the proximal left anterior descending coronary artery that was moderately calcified and non-tortuous.A 3.0x38mm xience skypoint stent was implanted.The following day, the patient developed ekg changes and chest pain.The stent had developed thrombosis and a dissection was observed at the distal end of the implanted stent.A 3.0x23mm xience skypoint stent was implanted later in the day as treatment.The patient had a good outcome with no adverse sequela.No additional information was provided.
