According to the reporter, the cuff was out.The line was exchanged on (b)(6) 2023.There was no reported patient outcome.

According to the reporter, prior to dialysis, the cuff was out.The cuff was still attached to line but line became dislodged.There was no leak and tego was utilized.There was no luer adapter issue and the catheter was not repaired.Chloraprep was the cleaning agent used on the device.Flushing was not performed prior to use.There was nothing unusual observed on the device prior to use and no other products were utilized with the device.There were no damages noted on the product.There was no securement wing issue andthe catheter did not move out of place due to securement wing issue.It was mentioned that the patient reported holding the line while asleep and waking up holding the line which might have contributed to the event.The catheter was maintained in place by sutures and dressing.Blood was taken, antibiotics and line was observed and replaced with a new device to resolve the issue.The line was exchanged on may 5, 2023.There was no blood loss and blood transfusion was not required.Antibiotic cover was the intervention/medical treatment required as a result of the event.There was no reported patient injury.
