Event Description
A physician was attempting to use an everflex entrust for treatment of a plaque lesion with 75% stenosis in the left superficial femoral artery (sfa).The degree of tortuosity and calcification was moderate.The device was prepped as per the ifu with no issues identified.No damage was noted to the packaging.The lock pin was checked for securement prior to the procedure.The lesion was not pre-dilated.No resistance during delivery to the lesion was noted and the device did not pass through a previously deployed stent.The lock pin was removed before usage of the stent.The stent would not deploy.The stent was removed with the guidewire.No intervention was required for removal of the device and was safely removed from the patient.The stent was exposed on the proximal part on removal.There were no issues with the guidewire used during the procedure.A new stent was used to complete the case.
