Event Description
|
A physician was using a hawkone during treatment of an 80mm calcified lesion with 50% stenosis in the mid left superficial femoral artery.The artery diameter was 5.8mm with moderate tortuosity and calcification.Up and over the aortic bifurcation was high with stenosis and it was challenging to get a sheath up and over.A 7fr non medtronic sheath was used and a 0.014 spider fx guidewire.A 7mm spider was used for embolic protection.The vessel was not pre-dilated.The ifu was followed. it was reported during the procedure resistance was felt during advancement. the physician performed treatment in common femoral artery first, device pulled back into sheath and it was accidently turned on briefly, when the physician attempted atherectomy on the superficial femoral artery portion of the vessel it was noted something was not right and the device was removed without complications and the cutter was noted to be damaged.The vessel was then ballooned.No patient injury reported.
