Event Description
|
A physician was attempting to implant an everflex entrust.The device was prepped as per the ifu with no issues identified.No resistance was met during delivery to the lesion and the device did not pass through a previously deployed stent.The lock-pin was removed before deployment.It was reported after the stent reached the target location deployment wasn't possible.The customer pulled back the stent and noticed that the stent was partially released and kinked.The customer inflated the stent afterwards, outside of the patient.No patient injury. .
