Event Description
It was reported that on (b)(6) 2023, a 23mm regent mechanical heart valve was chosen for implant for a patient undergoing aortic valve replacement (avr) procedure.It was reported after the valve was implanted, it was noticed the leaflets had "imperfect movement" and had difficulty moving freely.It was reported only the leaflet tester was used to test leaflet mobility and no other instrument or tool came in contact with the leaflets.A decision was made to explant and replace the 23mm valve with a 21mm regent mechanical heart valve.The replacement 21mm valve was successfully implanted in the patient.The 23mm valve was not tested for leaflet mobility after explant.It was reported there was a significant delay in the procedure due to this event.The patient remained hemodynamically stable throughout the procedure.The patient status was reported as stable.
 
Manufacturer Narrative
An event of the valve leaflets being unable to open and close properly was reported.Morphological and hydrodynamic examination indicated the valve met abbott specifications.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met all specifications.Hydrodynamic testing upon return to abbott and at the time of manufacturing indicated the valve functioned normally.This test ensures proper leaflet coaptation and hemodynamic performance.The cause of the reported event could not be conclusively determined.
