Event Description
According to the reporter, during a procedure, surgeon stated that the forceps did not make an adequate seal and after sealing the tissue remained bleeding.No re-grasp alert, there was an end tone and energy delivered.The device team did not report incomplete seal.

Event Description
According to the reporter, during extended abdominal hysterectomy with lymph node dissection, surgeon stated that the forceps did not make an adequate seal.No re-grasp alert, there was an end tone and energy delivered.About four not large vessels were sealed before the problem occurred.Cleaning was frequent, whenever necessary.The uterine arteries about 4mm approximately bled after sealing and surgeon controlled it with the suture.It was not extensive bleeding, so no blood transfusion was needed.The device was no longer used, the procedure was performed with conventional bipolar forceps and suture.The team did not report incomplete seal.A review was carried out by the technical service, who have already measured the powers delivered by the equipment and are fine.
