The patient (pt) stated the implantable neurostimulator (ins) has not been working for several years.Patient stated going to (b)(6) in (b)(6) thinking patient was going to get battery change.The healthcare provider (hcp) came out with a manufacturer representative (rep) and that he was not going to do the surgery because the battery is not dead like the hospital in (b)(6) in (b)(6), (b)(6) said.Caller stated they do not remember the year but they took the rechargeable battery out and put in non- rechargeable and patient did not think it worked since.They finally got the battery turned on after 3 tries and it had 2 years life still on it.Patient stated they needed the numbers for the settings.Patient being back and forth since april because a rep was going to go to kelly nelsons office the primary cares office april 24 in bridgeport, wv.Patient stated no one has called.Patient just spoke to hcp assistant at office and now they want patient to go to a different hcp in (b)(6).Patient stated not being able to go there and cant drive there.Patient stated tremors are so bad at times having trouble eating.
