Event Description
|
It was reported that the patient presented for an implant procedure.During the procedure, left ventricular lead failed to advance over the guidewire.The physician pushed the left ventricular lead to the target area until it could be placed.However, the lead exhibited failure to sense, high pacing impedance and high capture threshold.The lv lead was not used and replaced.There were no patient consequences.

The reported events of failure to sense, unacceptable capture, high pacing impedance and failure to advance guidewire were not confirmed.Final analysis found that as received, a complete lead was returned in one piece.Visual and x-ray examination of the lead did not find any anomalies.Electrical testing did not find any indication of conductor fractures or internal shorts.A guidewire was able to be fully inserted and removed from the inner coil with no obstruction/restriction.S-curve was measured to be within specification.
