It was reported that the cardiac resynchronization therapy defibrillator (crt-d) delivered a shock for atrial fibrillation (af) with rapid ventricular response.It was also noted that the left ventricular (lv) lead exhibited high thresholds.The crt-d and lv lead remain in use. no further patient complications have been reported as a result of this event.

|
Additional information received from the physician office noted that the patient was inappropriately shocked by the device forty-four times.Therapies were turned off per the patient request and approved by the physician.The patient also developed myalgias in the shoulders that has since resolved with medication and a feeling of exhaustion.It was further reported that there was a malfunction of the right ventricular (rv) lead.It was noted that the rv lead was not working well due to a loose set screw.
