It was reported that the patient with this cardiac resynchronization therapy defibrillator (crt-d) device received anti-tachycardia pacing (atp) and 6 shocks.The therapy was exhausted.The health care professional suspects that it could be ectopy.Upon review, it was determined that there were different morphologies and doesn't appear that the last shock was not converted.Boston scientific representative stated that defibrillation threshold (dft) test was performed, and it converted the rhythm.The plan is to continue monitoring and the patient is currently under medication.This device remains in service.No adverse patient effects were reported.
