Event Description
It was reported that the patient was in the emergency room due to experiencing atrial fibrillation (af) with rapid ventricular response.The patient's device met detection for ventricular tachycardia (vt), and delivered anti-tachycardia pacing and high voltage therapy.It was determined that a short circuit protection was triggered during several high voltage therapies and less than programmed high voltage energy was delivered.The cardiac resynchronization therapy defibrillator (crt-d) shocks did not convert the rhythm, but the rhythm spontaneously converted.It was also reported that there was a potential insulation issue with the right ventricular (rv) lead and an impedance warning was triggered for a low defibrillation impedance.It was also noted that oversensing and noise were observed on any lead configuration involving the rv coil.The crt-d and the rv lead were explanted and replaced.No further patient complications have been reported as a result of this event.

Event Description
It was further reported that the shocks were appropriate as the patient was in a ventricular arrythmia at the time of the shocks.The patient was admitted to the intensive care unit (icu).
