Event Description
The event occurred on an unknown date involving a 32" (81 cm) appx 6.3 ml, 15 drop admin set w/0.2 micron filter, rotating luer w/filter cap, bag hanger where it was reported that air was noted from the filter area all the way to the pump with residual medication in tubing leading from filter to the bottom of the bag.Back priming performed three times to attempt to remove air, but air does not go pass the filter area.The patient refused to wait for the secondary line to be sent back to pharmacy for priming.The patient requested to be discharged.The medication involved was etoposide 506ml/hr.There was patient involvement but no report of adverse reaction or need for medical intervention.

Additional information in d9 and h6.The complaint of product incorporates / allows air passage on item b30183 could not be confirmed by investigation.Since no product samples, pictures, or videos were received for investigation.Without the return of the used sample a comprehensive failure investigation cannot be performed and a probable cause cannot be determined.The device history report (dhr) lot# 9930806 was reviewed and no non conformities were found that would have led the reported complaint.
