Event Description
It was reported this was a mitraclip procedure to treat mitral regurgitation (mr) with a grade of 4.Prior to the removal of the dilator, the steerable guide catheter (sgc) hemostasis valve flush port broke.The sgc was replaced and the procedure continued without issue.The mr grade reduced to 1.There were no adverse patient effects and no clinically significant delay in the procedure.
 
Manufacturer Narrative
In this case the returned device analysis confirmed the reported flush port crack.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar complaints reported from this lot.Based on available information and without the device to analyze, the reported flush port crack was determined to be due to a potential product issue.Abbott will continue to trend the performance of these devices.There is no indication of a product quality issue with respect to manufacture, design, or labeling.
