Event Description
According to the reporter, at end of the procedure when turning the patient, there was a burn in the patient¿s arm and that the support bracket was wet and it was noted that there was component loose.Photograph was received and showed that there were blisters on the affected area.

Event Description
According to the reporter, during spinal arthrodesis between l5 - s1, the generator with the serial number (b)(6) had a component loose between the bipolar connector and the equipment connector requiring repositioning.The surgeon asked to change the unit to (b)(4).Upon turning the patient for extubating, there was a burn observed on patient¿s upper limb and that the arm support bracket was wet.The burn was not caused by handpiece and rem pad.Photograph was received and showed that there were blisters on the affected area.
