A customer reported error messages ¿2300 s.Loader step feed failure and 5100 assay operation aborted!¿ on the aia-900 analyzer.The customer cleaned the sample loading area, performed all-set-home tasks and rebooted the analyzer, but the errors persisted.The analyzer is down.A field service engineer (fse) was dispatched to address the reported event which resulted in a delayed reporting of patient samples for alpha-fetoprotein (afp).There is no indication of any patient intervention or adverse health consequences due to the delay in reporting of patient results.

A field service engineer (fse) was at the customer¿s site to address the reported event.Fse confirmed the complaint.While troubleshooting, fse found the loader was out of alignment when checking the sensor flag position and suspects it was possibly due to a sample rack that crashed.The fse realigned the sample loader sensor flag for x1 axis and performed sampler loader rotation on system mainte program function with no errors.Fse repaired and validated the analyzer by successfully performing the daily check, and quality control run without error and within acceptable range.No further action required by field service.The aia-900 analyzer is functioning as expected.A 13-month complaint history review and service history review for similar complaints was performed for serial number (b)(6) from 17jul2022 through aware date (b)(6) 2023.There were five similar complaints identified during the search period including this case for error message 2300 s.Loader step feed failure, however, there was none for error 5100 assay operation aborted.The aia-900 operator's manual under section12: error messages states the following: (2300) s.Loader step feed failure cause: the pitch sensor s071 failed to go on after the step feed.Action: check to see if there is any impediment to the sample rack feed.If the trouble reoccurs, contact the tosoh local representatives.Check s071 and the step feed mechanism.(5100) assay operation aborted cause: measurement was interrupted due to the occurrence of a phenomenon that prevented the continuation of measurement.Action: check the abort cause.The most probable cause was due to misalignment of the sample loader sensor flag for x1 axis.
