Event Description
It was reported that a small volume folfusor did not infuse.The device line did not have any kinks and the patient's peripherally inserted central catheter (picc) line was patent.The issue occurred during patient infusion.The device was filled with vancomycin (viatris) 1000mg in sodium chloride 0.9%.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

The device was connected to the patient for 24 hours prior to being disconnected.The actual device was received for evaluation.The unit did not contain fluid in the bladder because the luer was not closed with a cap, which allowed fluid from the bladder to empty inside a bag that contained the unit.A visual inspection did not identify any abnormalities that could have contributed to the reported condition.A functional flow rate test was performed and the flow rate of the device was found to be within specification.The reported condition was not verified.The device was determined to be conforming product.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
