|
Medtronic received information that during the implant of this transcatheter bioprosthetic valve, a non-medtronic guidewire (safari) was set in the left ventricle (lv).A sentrant 14 fr sheath was used to prepare the femoral artery, along with various delivery catheter system (dcs) guides without any problems.The dcs was then used with the non-medtronic guidewire, and felt strange to the physician, as the wire was exiting out the rear of the catheter.According to the physician, the guidewire was not kinked.The physician tried to forcefully advance the dcs in the femoral artery, but it was impossible to move forward or backward.The physician made the decision to cut the wire in order to remove the dcs from the patient.In order to retrieve the dcs out of the femoral artery, the wire was cut just ahead of the nosecone of the dcs.The dcs and wire were then retrieved out of the patient.The other portion of the wire remained in the body.The remaining wire was held with surgical "pince".A procedural delay due to the wire and dcs issue was reported, however the duration of the delay was not reported.The physician once again inserted the external sheath (sentrant), and replaced the wire, dcs and valve to complete the procedure.No adverse patient effects were reported.

Additional information was received that during the implant of the valve, the non-medtronic guidewire (safari) was cut between the skin and the delivery catheter system (dcs) as they remained out of the body together.The introducer sheath was reinserted on the non-medtronic guidewire that stayed in the patient.The non-medtronic guidewire was then exchanged for a new guidewire.
