Event Description
|
According to the reporter, the capsule failed to detached from the delivery system and therefore did not attach to the patient¿s esophagus.The doctor did not have to break the handle; the entire catheter was removed with the capsule still attached.Different delivery device was used and the capsule detached successfully and was confirmed with the endoscope view.There was no patient or user harm.
