Event Description
A turbohawk plus was being used during treatment of a 40mm calcified lesion with 90% stenosis in the proximal left region of the lef t superficial femoral artery.The artery was 7mm in diameter with mild tortuosity and severe calcification.The device was inspected and prepped as per the ifu with no issues identified.It was reported upon performing the first cut, the cath lab technician was un able to pack the device.After pulling the device back into the sheath, the technician was still unable to pack the device.The physician then decided to remove the device from patient.As soon as the device exited the sheath, the physician noted that the nosecone was coming off, and when it was removed from the wire, the nosecone detached from the catheter.
