Event Description
It was reported that during a port placement procedure via the left brachial vein under fluoroscopy, the introducer needle was inserted via the left brachial vein, when attempting to remove the guidewire, the tip of the guidewire got stuck in the chiari network making it difficult to remove.It was further reported that after repeated pulling of the guidewire, the resistance disappeared and it became possible to remove it.It was also reported that after the guidewire was removed, a cut chiari network with a length of 7 cm was found wrapped around the tip of the guidewire.There was no reported patient injury.
 
Event Description
