Event Description
It was reported that during the implant procedure of the implantable pulse generator (ipg) with epicardial lead, high thresholds were observed on the right ventricular (rv) lead.The epicardial lead was explanted and replaced but issue persisted.Suboptimal connection of the lead in the ipg was observed.It was also noted that the wrench screw driver was damaged, it became unstable and moving from its holder. ipg was explanted and replaced and the issue was resolved. no patient complications have been reported as a resultof this event.
