Event Description
A physician was attempting to implant an everflex entrust for treatment of a lesion in the mid superficial femoral artery.There was mild tortuosity and no calcification.The device was prepped as per the ifu with no issues identified.The lock-pin was checked for securement prior to the procedure.The lesion was pre-dilated with a 5.0x150mm pre-dilation device.The device did not pass through a previously deployed stent and no resistance was med during advancement.The lock-pin was removed when the stent was in position.It was reported during deployment an abnormal sound was heard from the delivery system and the physician tried to remove it, by doing this the hydrophilic guide broke and detached, the physician disassembled the release handle, he could not release the stent and finally he pulled everything out to remove it from the patient since the stent was not released from the system.No patient injury was reported.
