It was reported that the patient with this cardiac resynchronization therapy defibrillator (crt-d) experienced a ventricular tachycardia (vt) episode.The device delayed therapy and according to the field representative, the rhythm accelerated into the ventricular fibrillation (vf) zone and the device delivered eight shocks.All therapy available was delivered.The patient was externally defibrillated twice, the second shock converted the rhythm.Additionally, boston scientific technical services (ts) noted far field oversensing that resulted in inappropriate atrial tachy response (atr) episodes at the beginning of the episode, however, ts confirmed this did not contribute to the delay in therapy.No additional adverse patient effects were reported.At this time, this device system remains in service.
