The device was returned to olympus for evaluation and the customer's allegation was confirmed due to the additional heat shrink found.The device evaluation also found the distal end adhesive to be worn, a hole in the switch button, the up angle and right angle not to specification, play in the up/ down angle, and a failed electrical safety test.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

The customer reported to olympus, the evis exera iii gastrointestinal videoscope experienced an issue with the control body assembly insertion tube at the distal end.The issue was found during maintenance.The device was returned for evaluation.During the device evaluation, an addition heat shrink was found in the side cover of the control body.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a definitive root cause of the anomalous piece of heat shrink tubing found under the control body side cover could not be determined, however, the issue was likely the result of an oversight during a previous repair.Olympus will continue to monitor field performance for this device.
