A field service engineer (fse) conducted a site visit and was able to confirm and reproduce the reported issue by running prime bf wash.The fse observed an audible noise and verified the skipping motor.The fse removed the tower from the analyzer to manipulate the motor manually.The fse replaced the bf syringe unit and completed a daily check with no recurring issues.The aia-360 analyzer is operating as expected.No further action required by field service.A 13 month complaint history review and service history review for similar complaints was performed for the serial number (b)(6) through aware date of event.There were two (2) similar complaints identified during the searched period, which includes this event.The aia-360 operator's manual under section 7-1: list of error messages states the following: error message: 4045 wash.Sy home overrun.Description: the bf syringe motor overran on the home side.Troubleshooting: turn the power off and on again.If this problem reoccurs, contact the service department.The most probable cause was due to failure of the bf syringe unit.

Event Description
|
A customer reported 4045 wash sy home overrun error on the aia-360 analyzer.The customer powered the analyzer off and on, performed an all set home, and attempted to prime bf washer, but error recurred.The technical support specialist (tss) had the customer verify the wash was on and there were no kinks in the tubing.The analyzer is down.A field service engineer (fse) was dispatched to address the reported event, which resulted in a delayed reporting of patient samples for estradiol (e2) and progesterone (prog iii).There is no indication of any patient intervention or adverse health consequences due to the delay in reporting of patient results.
