(b)(4).This follow-up report is being submitted to relay additional and/or corrected information.Visual examination of the returned product confirmed that the item and lot numbers match that which is on the complaint.The instrument looked well used and featured multiple dents and scuffs to its surfaces.It was confirmed that the instrument exhibited a fracture in the area of the item number and the broken off section of the bearing was not returned for evaluation.A review of the device manufacturing records confirmed no abnormalities or deviations that could be related to the reported event.Device is used for treatment.Medical records were not provided.With the available information, a definitive root cause could not be determined.If any further information is found which would change or alter any conclusions or information, a supplemental report will be filed accordingly.Zimmer biomet will continue to monitor for trends.
