This is report 3 of 3 for(b)(4).It was reported by the healthcare professional in china that during a meniscal repair procedure on (b)(6).2023, it was observed that the plate on the truespan 24 degree peek device could not fix the meniscus at three different suture positions after deployment.Changed to another two to continue the surgery but the same problem happened again.Another like device was used to complete the procedure.There were no adverse patient consequences nor surgical delay reported.No additional information was provided.

This report is being submitted in pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by mitek or its employees that the report constitutes an admission that the device, mitek, or its employees caused or contributed to the potential event described in this report.Device was used for treatment, not diagnosis.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.H10 additional narrative: investigation summary: the product was returned to mitek for evaluation.Mitek then conducted visual inspection of the device received.The complaint device was received and evaluated; visual inspection reveals that the device had the trigger broken, it was opened to verify the broken part, it was found that the upper part of the red trigger is broken.The needle was found to be noticeably bent, also one plate was found still attached to the applier needle.The first plate and suture were not returned.A manufacturing record evaluation was performed for the finished device lot number and no non-conformances were identified.As part of depuy synthes mitek quality process all devices are manufactured, inspected, and released to approved specifications.Based on one of the plates was found still attached to the applier needle, this complaint was confirmed.The possible root cause can be attributed to a portion of tissue that was entered inside the applier needle causing a mechanical obstruction of the first plate when it was going to be deployed; this obstruction and the force applied to the trigger are contributive factors of the broken trigger, therefore the plate was not released.A possible root cause for the bent applier needle can be attributed to procedural variables, such handling of the device or product interaction during procedure; an excessive manipulation of the device, tilting movements of the applier needle while it was inserted causing the needle to bent, however this cannot be conclusively determined.At this point in time, no corrective action is required, and no further action is warranted.However, depuy synthes mitek will continue to monitor additional complaint information for potential safety signals through complaint trending as part of post market surveillance.
