It was reported that this cardiac resynchronization therapy defibrillator (crt-d) potentially delivered inappropriate therapy for an atrial driven rhythm.Technical services (ts) reviewed the reports and noted that anti-tachycardia pacing (atp) and shock therapy was delivered, and the rhythm accelerated.A shock converted the rhythm.Additionally, ts stated that there was a pacemaker mediated tachycardia (pmt) episode that appeared to be atrial driven.Also, there was a signal artifact monitor (sam) episode due to atrial fibrillation (af) on the atrial channel that resulted in a vector switch.The device remains in use.No additional adverse patient effects were reported.
