Event Description
|
It was reported that during an implant procedure, this patient's previously existing competitor left ventricular (lv) lead was noted to be difficult to insert into the header of this new cardiac resynchronization therapy defibrillator (crt-d).After troubleshooting, the crt-d was eventually able to be connected with the lv lead and the implant procedure was completed successfully.The crt-d remains in service and there were no adverse patient effects reported.

This product has not been returned to boston scientific, and as a result, laboratory analysis could not be conducted.The associated investigation noted evidence indicating difficulty may have been encountered fully inserting an is-1/df-1 lead into the header of this device.Please refer to the description for more information regarding the specific circumstances of this event.
