Event Description
During an implant procedure, the guidewire was unable to be advanced into the left ventricular (lv) lead.The lv lead was explanted and replaced to resolve the event.The patient was stable and will continue to be monitored.
 
Manufacturer Narrative
The reported event of failure to advance the guide wire was confirmed.As received a complete lead was returned in one piece for analysis.Visual examination and the guidewire test found the inner coil clogged with foreign material consistent with material most likely occurring during time of procedure.The cause of the reported event was isolated to the foreign material blocking the inner coil.
