Device report from synthes reports an event in china as follows: it was reported that during the surgery, when insert the screw, the screw's was broken, all the pieces were removed from the patient.Another device was used to complete the surgery.There was no surgical delay.There were no adverse consequences to the patient.The patient current status and outcome: stable.No additional information could be provided.This report is for one (1) cranial-scr plusdrive ø1.6 self-drill l4 this is report 1 of 1 for complaint (b)(4).

Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.H3, h4, h6 the product was returned to depuy synthes for evaluation.The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that the cranial-scr plusdrive ø1.6 self-drill l4 had the edges of the threads screw worn, the distal tip was noted broken and the shaft slightly bent.The device was observed with white unknown particles attached to shaft, these conditions mostly can be caused by usage / attempted, hard bone or misalignment of the screw during the procedure and cannot be traced to a manufacturing issue.In addition, the broken fragment was not received.A dimensional inspection for the cranial-scr plusdrive ø1.6 self-drill l4 was not performed due to post manufacturing damage.The observed condition of the device was consistent with a random component failure that may have been caused by exposure to unintended forces.As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.The overall complaint was confirmed as the observed condition of the cranial-scr plusdrive ø1.6 self-drill l4 would contribute to the complained device issue.There is no indication that a design or manufacturing issue has caused the complaint condition and hence the root cause cannot be determined.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Drawing/specifications reviewed the following source controlled drawings reflecting the current and manufactured revisions were reviewed: 1.6mm cranial slf-drlg screw cruciform, low profile 400_834_susa rev.D (current and manufactured).Dimensional inspection: n/a.H4, h6 a manufacturing record evaluation was performed for the finished device product code : 400.834.04s.Lot number# 5659p75.It was electronically reviewed and no nonconformances / manufacturing irregularities were identified during the manufacturing process.The product was released on: 02/05/2023.Manufacturing site:jabil bettlach.Expiry date:01/04/2033.Device was used for treatment, not diagnosis.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
