|
A customer reported sc flags on quality control (qc) and samples on the aia-360 analyzer.The analyzer is down.A field service engineer (fse) was dispatched to address the reported event, which resulted in a delayed reporting of patient samples for beta human chorionic gonadotropin (bhcg), estradiol (e2), and progesterone (prog iii).There is no indication of any patient intervention or adverse health consequences due to the delay in reporting of patient results.

A field service engineer (fse) conducted a site visit and was able to confirm the reported problem by reviewing the patient and quality control (qc) printouts.The fse verified the sampling nozzle was free from clogging material and verified clog sensor fittings were tight.The fse replaced the sensor board and adjusted clog sensing circuit.The fse validated the analyzer by running quality control with results within specification.The aia-360 analyzer is functioning as expected.No further action required by field service.A complaint history review and service history review was performed for serial number (b)(6) from the install date through the aware date.There were two (2) similar complaints identified during the searched period, which includes this event.The aia-360 operator's manual under section 7-2: list of flags states the following: sc: dispensing was cancelled because clogging was detected during sample suction.Check that the sample is free of fibrin or other substances.If the sample is not clouded, the sample nozzle may be faulty or the piping may be blocked.If this problem occurs frequently, contact the service department.The most probable cause was failure of the sensor board.
