Event Description
During a transcatheter aortic valve replacement procedure, a cardiac perforation occurred which required surgical repair to stabilize the patient.After venous access was obtained, the physician advanced the catheter up to the right ventricle.It was positioned at the apex of the right ventricle and balloon was deflated.The catheter was then successfully tested for conductivity.The perforation was noticed at the apex of the right ventricle while examining the newly placed valve under ultrasound.No patient symptoms were noted.There were no reported performance issues with any abbott device.

The results of the investigation are inconclusive since the device was not returned for analysis.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.Based on the information received, the cause of the reported incident could not be conclusively determined.Per the ifu, cardiac tamponade and pericardial effusion are known risks during the use of this device.
