Event Description
|
A customer reported error message "2236 bf probe 2 suction failure" on the aia-900 analyzer.The customer was instructed by a technical support specialist (tss) to replace the bf tips and clean the surrounding area as well as check the waste tubing.The error persisted.The analyzer is down.A field service engineer (fse) was dispatched to address the reported event, which resulted in a delayed reporting of patient samples for beta human chorionic gonadotropin (bhcg), estradiol (e2), follicle stimulating hormone (fsh), luteinizing hormone (lh ii) and progesterone (prog ii).There is no indication of any patient intervention or adverse health consequences due to the delay in reporting of patient results.

A field service engineer (fse) conducted a site visit and was able to confirm the problem by reviewing the error log.The fse ran bf wash and checked wash probes (s132) and (s133).The wash probe (s132) led was flickered during performing wash prime.The fse replaced wash probe (s132) and resolved the complaint.The fse verified the resolution by successfully running controls.No further action is required by field service.The aia-900 analyzer is functioning as expected.A 13-month complaint history review and service history review through aware date of event for similar complaints was performed for serial number (b)(6).There were no similar complaints identified during the search period.The aia-900 operators manual under section 12: flags and error messages state the following: [2236] bf probe 2 suction failure cause: the overflow sensor (b)(6) detected liquid after a washer suction operation.A wu flag will be attached to the measurement result.Action: clean the wash probe.Check s133, the waste liquid solenoid valve sv171, the waste liquid tube, and the liquid pump lp173.The most probable cause of the reported event was due to faulty wash probe (s132).
